Please attach four copies of the proposed recital program, typed or printed on an 8.5 x 11 inch paper. Please list complete information, including title, key, opus number, movement, time length, and composer. If a selection is a part of a larger work, please indicate the name of the larger work (i.e., Il Trovatore, Sonata in A Major, etc.). The name of the performer should appear only at the lower right hand corner of the sheet.

The winner(s) should be prepared to submit two glossy print photos (preferably 5 x 7) to be used for publicity.

There is a non-refundable application fee of $15.00 for an individual or for an ensemble.  The teacher need not be a contributor to The Friends of Music. You are, of course, encouraged to become a contributor.
